2024-2025 Calendar

      To register as a parishioner of Blessed Sacrament Church, please go to our parish website at https://blessedsacramentchurchsi.org   You can begin the registration process for our Faith Formation Program by calling our office (718-448-0378) for an appointment, or pick up a registration form at the Parish House, 30 Manor Road.  After completion of the application, kindly return the application to the Parish House for processing by our office.        

            Fees – Registered Parishioners – one child $185.00; two or more children in the same family $230.00.  Bus service from PS 30 $185.00; and New Registration Fee of $50.00.  Cash and/or checks will be accepted; make checks payable to Blessed Sacrament Faith Formation.

            If your child is an incoming 1^st grader, you must submit a copy of your child’s Baptismal Certificate.  If your child is a transfer student from a Catholic school, public school or from another religious education program, you must submit a copy of that child’s Baptismal Certificate and for an older child, the date of Penance and a copy of the First Eucharist Certificate, as well as a transfer note from their previous school.
